Note to Current Authorized Providers
If you require items from our office, you must allow us a minimum of 5 days
to prepare. If you request items too late, we may not be able to get you
everything you requested.
Also, in order for us to issue your cards quickly please adhere to the following:
- Course Records and Addendums must be written clearly to prevent errors on cards.
- Fill in demographics section of each course record.
- Fill in number of hours for each class.
- Use the appropriate code for the class taught. If you do not know the correct
codes for your class, you can obtain a copy of
course codes here, or pick one up at our office.
